The blood–brain barrier: implication in the stroke pathophysiology

La barrière hémato-encéphalique joue un rôle crucial dans la préservation du système nerveux central en régulant sélectivement les échanges moléculaires entre la circulation périphérique et le parenchyme cérébral. Composée de diverses composantes cellulaires et de jonctions serrées, cette barrière assure la protection et la stabilité du microenvironnement cérébral. Cependant, les accidents vasculaires cérébraux compromettent l’intégrité de la BHE, altérant ses fonctions vitales. Cette revue décrit la structure et les fonctions de la BHE, mettant en lumière sa vulnérabilité lors des AVC incluant une composante inflammatoire. Enfin, nous discutons des stratégies thérapeutiques susceptibles de restaurer le fonctionnement de la BHE post-AVC et des techniques d’imagerie permettant d’évaluer son intégrité in vivo.

The blood–brain barrier plays a crucial role in preserving the central nervous system by selectively regulating molecular exchanges between the peripheral circulation and the brain parenchyma. Comprised of various cellular components and tight junctions, this barrier ensures the protection and stability of the cerebral microenvironment. However, strokes compromise the integrity of the BBB, affecting its vital functions. This review describes the structure and functions of the BBB, shedding light on its vulnerability during strokes, including an inflammatory component. Finally, we discuss therapeutic strategies aimed at restoring the functioning of the BBB post-stroke, along with imaging techniques to assess its integrity in vivo.
